Schedule Fee
Psychologist fees are based on a 50-minute hour.
Billable services include the following: face-to-face sessions, telephone consultations, video conferencing, report writing, supervision of other team members, programming, email correspondence (other than scheduling), report writing or other requested written documentation, review of written records/reports from other professionals, consultation with other professionals, and travel if services are delivered in home, school or community settings.
Payment Process
Payment for services is due before or at the time of your scheduled appointment via Etransfer. Please send Etransfers to the following email address: drstaceytomanik@gmail.com. If services are being sought for your child, please use the child’s first name as the answer to the security question (“What is my child’s first name?”). If you are seeking services for yourself, the answer should be your first name.
Please note that we do not direct bill insurance companies. Clients are encouraged to contact their insurance provider prior to appointments to obtain information about their coverage. You will be provided with a receipt following your appointment to submit to your insurance company for reimbursement.
Deposit
We require a $1000 deposit for all psychological evaluations. The deposit is due at the time of booking, prior to your first appointment. You will be provided with an estimate of the cost of the assessment at the time of booking. The balance for the assessment is due prior to the feedback session and before the report is released.
Cancellations
If you are unable to attend your scheduled appointment time or wish to reschedule your appointment, please notify us via email or telephone as soon as possible. A minimum of 24-hours notice is required for all cancellations. Cancellations that are received without 24-hours notice will be billed at the cost of the session, without exception. If you fail to attend a scheduled appointment you will be billed for the full cost of the session.
Office Hours
Our office hours are 9:00AM to 5:00PM, Monday to Friday. Evening appointments may be available on request.
Emergency Assistance
In the event of an emergency, please contact the Distress Centre at (403) 266-4357. The Distress Centre offers free crisis counselling and 24-hour telephone support. You may also contact your physician or call 911 if appropriate.
